Payment Options

No insurance? No problem. At Elevated PT we offer four payment options that do not go through insurance. We make high quality physical therapy attainable through transparent pricing and not allowing insurance companies to dictate your treatment plan.

Self-pay

We accept credit cards, debit cards, check, cash, Venmo, Zelle, and Paypal.

A $5 discount off each visit is given when payment is made with cash or check.

HSA/FSA

If you have an HSA/FSA card you can use your benefits to pay for Elevated PT services.

Remember, the money on an FSA card expires at the end of the year. The money on an HSA card continues to accumulate until you ask for reimbursement and provide a receipt for the services.

Elevated PT can provide you with either a simple receipt or a superbill receipt upon request.

Superbill

A superbill is a receipt that includes the specialty codes (CPT codes) used to describe to your insurance company what services were provided during your visit.

Your insurance company will then reference their determined worth of each CPT code and reimburse you based on that criteria. This means you may or may not be reimbursed the full amount for your sessions.

You will need to call your insurance company and confirm with them that they will reimburse you for out-of-network services if you provide them with a superbill.

Payment plan

Elevated PT has partnered with Cherry to offer our clients payment plan options.

You will need to do a soft credit check.

There is a 6 month 0% APR paln option, but you can extend your payments beyond this time period.

You must apply to know what your options are. Estimates made before you apply with Cherry are just estimates.
